REGISTER FOR 2025 SHAARAI DAY CAMP

We are looking forward to sharing a week with your child at Shaarai Shomayim's Summer Day Camp!
 

Registration Guidelines:

• Preschoolers are Half-Day Campers and 1st - 7th graders are Full-Day Campers. Parents of Kindergartners (entering or just finished) have the option to choose Half- or Full-Day.
• T-shirts are guaranteed to all campers who are registered and have paid in full by May 1.
• All children must be potty trained in order to attend Shaarai Day Camp.

EMERGENCY CONTACT AND MEDICAL RELEASE INFORMATION:

In the case of an emergency, please complete the emergency contact information below for your child(ren). By completing this, you are agreeing to the following:

I permit my child to attend Shaarai Day Camp and ride a school bus to approved ‘Camp’ activities.

In addition, if an injury requires medical attention, I grant permission to the supervising teacher(s) or staff (including volunteers) to attend to my son/daughter. If the injury warrants further medical attention, I expect every effort to be made to contact me to receive my specific authorization before action is taken. If efforts to contact me are unsuccessful, I grant permission for necessary medical treatment to be given. In addition, I consent to the supervising teacher(s) or staff (including volunteers) taking my child to the physician, dentist, or the hospital if an accident or serious illness occurs at ‘Camp’ and I cannot be located.

Know that in an emergency, we may need to access the closest care hospital.

Further, I hereby release Congregation Shaarai Shomayim (including its members, directors, officers, employees, teachers, volunteers, school program, agents, and assigns) from and against any claims, demands, actions, complaints, suits, or other forms of liability arising from any damage, injury or death caused by or to my child in connection with my child’s participation in Shaarai Day Camp, including participation in any Camp activity or bus ride.  I also agree to indemnify and hold harmless the foregoing parties from such released claims, including any related costs, attorney fees, liabilities, settlements, and/or judgments.
