In this section, please include any allergies, dietary needs, medical conditions, health considerations, learning issues, physical accommodations, any accomodations made in secular school, anything else you think we should know in order to successfully educate your child (this information is only shared with the classroom teachers, if deemed necessary, and is otherwise confidential).

These can be other congregants, other school parents, nearby family members or neighbors...who will assume temporary responsibility for your child in the event that we cannot reach you. Please include names and phone numbers.

In case of accident or serious illness, I request that the school contact me. If the school is unable to reach me, I hereby authorize the school to call the physician named above and to follow his/her instructions. If the physician cannot be reached, the school may make whatever arrangements are deemed necessary and appropriate.
